It has never been more important to stay on top of your good health, especially if you have a chronic condition such as congestive heart failure, COPD, diabetes or high blood pressure. We are here to help!
We are CareVio™, a network of clinical experts — nurses, care coordinators, pharmacists, social workers, respiratory therapists working with a medical director — working together with your Primary Care Team to help coordinate all of your health care needs. Our friendly, knowledgeable CareVio team members can help:
- Support you in understanding your chronic health condition, including information on how to manage your own care and when to reach out to your Primary Care Team.
- Coordinate your medical care with all members of your health care team.
- Answer questions about your medical condition, the medicine you should take, and the lab tests your health care provider orders for you.
- Connect you with community resources for life’s challenges that may make it harder for you to get or stay healthy.
eBrightHealth ACO, an accountable care organization in partnership with your care team is working hard to make sure you have the care and services you need to stay as healthy as possible, CareVio assistance is available at absolutely no cost to you.
You may soon receive a call from a CareVio care coordinator, who will quickly become a trusted expert you’ll turn to again and again with questions about your health. CareVio is part of your Primary Care Team, so be assured that your medical information stays secure and completely confidential.
